Payment for Services Rendered

We appreciate your business and the opportunity to serve you. Fees are set during the initial visit and payment is expected at the start of each session.

INSURED CLIENTS:

We currently accept limited insurance plans and will bill your visit directly to your insurance provider if you supply an updated proof of insurance at each visit. Please note that the insurance policy is between you and your insurance provider; we are happy to file the claim for you, but the balance is your responsibility if your plan does not pay after 30 days. Any amount not covered by your insurance policy is due immediately after you receive your first invoice from us.

OUT-OF-POCKET AND UNINSURED CLIENTS:
We accept cash, VISA, Mastercard, and Discover. Payments can be made online by going to http://www.professionalcharges.com/ and clicking on 'Make a Payment' under the 'For Clients' section and entering Angela's license number PC0000438.
